Introduction

books on how to treat psychiatric illnesses."
Most doctors have the training in interviewing a pauent ana most aoctors
are doing it well. Also general practitioners are at the front-line when it
comes to family, couples or personal problems. Often referrals to
psychiatrists leave a time gap between the date of referral and the date
the patients' problems are actually dealt with. GPs are the best persons to
deal with patients' problem, be these physical or psychological or mixed.
I have been juggling time between consultations and counselling for the
last 12 months. I think if one can plan a little bit ahead and have some
guidelines, the juggling can be done rather successfully. At the end, it is
the patients who will benefit from having their problems dealt with
sooner and we, as general practitioners, can have the satisfaction of
helping our patients while not straining on our income.
That was the dilemma I faced in the early 1990s. I began to see that a lot
of the patients' problems had a psychological overtone. No matter how
hard I tried to deal with their physical symptoms, how many tests and
how many specialists referrals, I seemed to get nowhere.
So I started to look into venues where I could learn skills in how to help
patients with psychological problems. At the time, counselling seemed to
offer me a chance to learn that skill. As I went further into it, it became a
case of the more I know, the more I know that I don't know.' As in
Medicine, pursue of knowledge and skill can be never-ending. However,
I think I am at the stage to offer you what I have learned and how you
can involve the skill in counselling to help your patients deal with the
immediate problems or situations.
As a busy general practitioner, it is most likely hard for you to find time
for counselling. So if you are going to incorporate some counselling into
your practice, it makes sense to identify the time when it is least busy in
your practice and also the time it suits your patient. However, it is
important to set a time right for yourselves first. Then the patients will
